What is Senior Care Advantage of Palmetto?
Palmetto Primary Care Physicians wants to help you reach your health and wellness goals. That’s why we developed a special program called Senior Care Advantage of Palmetto. This program is not a new Medicare Advantage plan. Rather, it is a collaborative approach to care delivery that focuses on preventive care and providing care coordination resources for individuals enrolled in participating Medicare Advantage plans. Through this program, the primary care providers and care teams at Palmetto Primary Care Physicians are able to offer services that focus on your total health.
Access to Services
Palmetto Primary Care Physicians’ patients enrolled in a participating Medicare Advantage plan are able to access the following services through the Senior Care Advantage of Palmetto program:
- Annual Medicare wellness visit
- Cancer screenings and preventive care visits
- Coordinated care with other health professionals and services
- Referrals to specialists and other health professionals
- Patient outreach about upcoming or needed appointments
- Care team support for chronic conditions
Options For Your Needs
Under this program, you have the flexibility to choose one of the participating Medicare Advantage health plans that we and Palmetto Primary Care Physicians work with. We contract with most major Medicare Advantage plans like Aetna, UnitedHealthcare and Humana.
If you are already a patient of Palmetto Primary Care Physicians enrolled in a Medicare Advantage plan with one of the participating insurance companies, you automatically have access to the Senior Care Advantage of Palmetto program. Please consult with your provider at Palmetto Primary Care Physicians if you are unsure.
